Red obtained Charizard as a Charmander from Professor Oak when he went out on his journey. It evolved into a Charizard sometime after Red defeated Koga between File 2 - Cubone and File 3 - Giovanni. During the battle against Mewtwo in File 4 - Charizard, Red's Charizard Mega Evolved into Mega Charizard X and battled Mewtwo. It was the first Mega Evolution seen in the anime. Great question!
