About this deal

People who used the service told us that staff respected their choices and that they were supported and enabled to do things independently. They confirmed that they were getting the support they needed and told us how they took part in the ongoing assessment and review of their needs and goals. They said they were involved in the recruitment and selection of staff and that staff had the right skills to support them. They told us about how they were encouraged to take part in the daily running of the service. Serenity House can accommodate up to three people in one adapted building. At the time of our inspection two people were living at the home. The care service has been developed and designed in line with the values that underpin the Registering the Right Support and other best practice guidance. These values include choice, promotion of independence and inclusion. People with learning disabilities and autism using the service can live as ordinary a life as any citizen. The last rating for this service was requires improvement (published April 2021). We issued a requirement notice in relation to regulation 12 (Safe care and treatment) and a Warning Notice and the provider was required to meet the requirements of regulation 17 (Good governance) by 31 July 2021. The service rating has deteriorated to inadequate. Breaches of legal requirements were found, and the service was placed in special measures. This service has been rated requires improvement for the last four consecutive inspections.

There were two registered managers in post. One for the residential home and another for the homecare service. A registered manager is a person who has registered with the Care Quality Commission to manage the service. Like registered providers, they are ‘registered persons'. Registered persons have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated regulations. We work closely with health insurance plans, Medicare, CenCal (MediCal) to maximize the portion that insurance covers. Serenity House room and board is not generally covered by Medicare or insurance. If qualified, subsidized care is offered on a sliding fee scale. Our charitable mission ensures that those who cannot afford are not turned away. In the residential service, we found people were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests; the policies and systems in the service supported this practice. During the inspection we recognised that the provider had failed to notify CQC of notifiable events. This was a breach of regulation.Staff had received training to support their role. Staff had started to receive supervision and plans were in place to ensure people received this on a regular basis.

We reviewed a range of records. This included two people’s care records and multiple medication records. We looked at two staff files in relation to recruitment and staff supervision. A variety of records relating to the management of the service, including policies and procedures were reviewed. to meet those needs. People had access to other healthcare professionals such as a dietician and GP and were supported to eat enough to keep them healthy. People had access to drinks during the day and had choices at mealtimes and where people had special dietary requirements we saw that these were provided for.We saw that staff obtained people’s consent before providing care to them. People did not have access to activities and community facilities.

By getting help at our residential detox center, you’ll see that you don’t have to do this alone. While in treatment, you’ll see that there are many other people dealing with similar issues, and they’ll be there as part of your support system, along with staff and counselors. This will allow you to see that you’re not alone, and you never have to be alone again. A complaints procedure was in place. There were appropriate management arrangements in place. Staff felt supported by the provider and assistant manager and staff meetings took place. Regular audits of the service were carried out to assess and monitor the quality of the service. The provider acted in accordance with the Mental Capacity Act 2005 (MCA) and Deprivation of Liberty Safeguards (DoLS). If the location is a care home the Care Quality Commission is required by law to monitor the operation of the DoLS, and to report on what we find. Contributions should be travel related. The most helpful contributions are detailed and help others make better decisions. Please don’t include personal, political, ethical, or religious commentary. Promotional content will be removed and issues concerning Booking.com’s services should be routed to our Customer Service or Accommodation Service teams. People told us they felt safe with the staff who supported them. Staff we spoke with had good knowledge of their responsibilities in relation to protecting people from abuse. Information in people’s care plans gave staff the guidance to provide effective support. People’s medicines were managed safely, and staff showed a good understanding of how to reduce the risk of infection for the people they cared for.Rating at last inspection: Requires Improvement (Report Published). At our previous comprehensive inspection in October 2017 the service was rated overall requires improvement. We found a breach of Regulation 17 HSCA 2008 (Regulated Activities) Regulations 2010.
