About this deal

Phenylephrine hydrochloride: Phenylephrine is absorbed from the gastrointestinal tract, but has reduced bioavailability by the oral route due to first-pass metabolism. It retains activity as a nasal decongestant when given orally, the drug distributing through the systemic circulation to the vascular bed of the nasal mucosa. When taken by mouth as a nasal decongestant phenylephrine is usually given at intervals of 4-6 hours. The NHS says that babies aged 3-5 months can have a maximum of 2.5ml of ibuprofen three times in 24 hours (3-4 times in 24 hours if they're aged 6-11 months). The recommended dose in this case is 10-20ml up to four times in 24 hours. Read the label if you're unsure. There's nothing to stop adults or children over 16 from using Calpol or other pain-relief medication intended for children. If your baby still needs this medicine two days after receiving the vaccine talk to your doctor or pharmacist (leaflet).

Tricyclic antidepressants (e.g. amitriptyline): may increase the risk of cardiovascular side effects with phenylephrine (see section 4.3). Sorbitol and maltitol may cause gastrointestinal discomfort and have a mild laxative effect. Each 5 ml spoonful of this product contains 0.97g sorbitol liquid and 2.72g of maltitol liquid. Calorific values: 2.6 kcal/g sorbitol and 2.3 kcal/g maltitol.

Treatment

This medicine contains less than 1 mmol sodium (23 mg) per 5ml, that is to say essentially 'sodium-free'.

Symptoms of paracetamol overdosage in the first 24 hours are pallor, nausea, vomiting, anorexia and abdominal pain. Liver damage may become apparent 12 to 48 hours after ingestion. Abnormalities of glucose metabolism and metabolic acidosis may occur. In severe poisoning, hepatic failure may progress to encephalopathy, haemorrhage, hypoglycaemia, cerebral oedema, and death. Acute renal failure with acute tubular necrosis, strongly suggested by loin pain, haematuria and proteinuria, may develop even in the absence of severe liver damage. Cardiac arrhythmias and pancreatitis have been reported.

Alcohol increases the risk of liver damage that can occur if an overdose of paracetamol is taken. The hazards of paracetamol overdose are greater in persistant heavy drinkers and in people with alcoholic liver disease. A wealth of other cold and flu remedies are available to buy without a prescription. Many of these will also contain paracetamol and phenylephrine in their ingredients. Caution should be taken when paracetamol is used concomitantly with flucloxacillin as concurrent intake has been associated with high anion gap metabolic acidosis, especially in patients with risks factors (see section 4.4) If you take paracetamol in the hour before or the six hours after taking colestyramine, the paracetamol is likely to be less effective. This is because colestyramine reduces the absorption of paracetamol from the gut. You should avoid taking Lemsip cold & flu sachets one hour before or six hours after taking colestyramine.

This medicine contains 14.32mg propylene glycol (E1520) in each 5ml dose, which is equivalent to 2.86mg/ml. Caution in babies less than 4 weeks old. Co-administration with any substrate for alcohol dehydrogenase such as ethanol may induce serious adverse effects in neonates.
